Please fill out this form entirely so that our factory representative can review your case, determine warranty eligibility, and contact you to schedule service. Please note, these requests are only monitored during our normal business hours of 8AM to 5 PM EST Monday through Friday. All units under warranty must be worked on by a Cleveland authorized service company. Work by unauthorized service agents may void the warranty. If you need service outside of regular business hours, please contact your local factory approved service provider.
